We are looking for an experienced Speech Language Pathologist. This is a part time, 32 hours per week position working between the hours of 8am-4:30pm, but may require flexibility with office hours. The Home Care office is located in York, Maine.
The Speech Language Pathologist is responsible for:
1. Providing speech therapy services to patients in their homes in accordance with a written Plan of Care approved by a physician.
2. The Speech Therapist makes an initial evaluation visit and plans with patient, family and physician for on-going Home Health services.
3. Initiates, reviews and implements the Speech Therapy Care Plan for each patient based on the assessment and prescribed Plan of Care.
4. Revises Plan of Care when indicated.
5. Initiates appropriate preventative and rehabilitative Speech Therapy procedures.
In order to help us continue to offer exceptional patient experiences, we need you to have the following:
1. Minimum of (1) year of Speech Language Pathology experience. Prefer Home Care experience.
2. Outstanding customer service.
3. Computer skills.
4. State of Maine SLP License
5. Basic Life Support (CPR) Certification
6. A valid Driver's License
