This is an exciting opportunity to join our expanding palliative support team, delivering excellent palliative and end of life care in patients homes.
A full UK driving license and access to a vehicle are essential for this post. Please note that an international driving license is not accepted.
The post holder will work as part of the Community Palliative Care Team and undertake a broad range of nursing support duties relating to the care of patients with end of life care needs in the place where they live, usually their own homes.
The post holder will work under the direction of the Palliative Care Team providing care as required to patients with palliative and end of life needs. The post holder will work within the clearly defined boundaries of their competencies.
Responsibilities
* Communicate with patients, carers, and health professionals in a professional manner.
* Establish and maintain effective communications with patients, carers, and health professionals.
* Update patient records ensuring entries are accurate, relevant, legible, and timely, and communicate care provided appropriately.
Requirements
* Have a basic understanding of the psychological and spiritual needs of patients and their families.
* Recognize own clinical limitations and seek help appropriately.
* Maintain IT competency in order to make full use of mobile phones, email, the internet, and on-line training.
