Main duties of the job
Our Healthy Child Programme teams offer a proactive approach to delivering the Healthy Child Programme, working in partnership with school nurses, early help staff at children and family centres and multi-agency teams to deliver the best outcomes for children. We are supported by forward thinking Practice Educator and Safeguarding Teams to deliver safe and effective care to our clients. The post holder will work within a skill mixed team to promote the health and wellbeing of children and their families.
The post holder will provide specialist practitioner knowledge and advice; assessing care needs, developing, implementing and evaluating programmes of care and public health developments in the community, liaising with other agencies as required. Practitioners at this level assume responsibility for planned teaching/supervision of junior staff and students, and will ensure the mentorship and supervision of students both pre and post registration.
The Crawley Hub is part of the wider HCP East 0-5 Team in West Sussex, one of 6 hubs across the county. Health visitors may be required to work anywhere in the Sussex Community Foundation NHS Trust area in order to meet the needs of the service.
