You will deliver high-quality care programmes for adults referred to the Frailty Hospital at Home service, providing support in patients' homes to prevent unnecessary hospital admissions.
This role requires excellent clinical assessment skills and the ability to holistically approach patient care, as well as a full UK driving licence for travel purposes within the community.
