This is an excellent opportunity for a practitioner with up to six years of post-qualified experience to develop their skills within a supportive and well-structured environment. You’ll be working with adults with a range of complex needs, undertaking assessments, managing risk, and coordinating support plans that promote independence, wellbeing, and safety. The role involves close partnership working with health professionals, housing, and community services to ensure person-centred, outcome-focused support.
