Job Overview
An exciting opportunity has arisen to lead on embedding an enablement approach within mental health social work. You will develop, implement and review a new approach to building the independence of people diagnosed with mental health issues who require social care services from Cambridgeshire County Council. This will involve reviewing current arrangements, designing and implementing service transformation, and identifying areas for improvement. You will work across Adults and Older Adults teams, including transitions, and collaborate with commissioners, contracts, providers, Early Help, TEC and health teams to develop an enabling approach for individuals.
The role requires reviewing individuals referred, carrying out an initial baseline assessment to determine their needs, and subsequently conducting reviews and benchmarking to assess changes in personal outcomes and level of care and support.
Key Responsibilities
* Review individuals referred and carry out initial baseline assessments.
* Work with commissioners, contracts, providers, Early Help, TEC and health teams to develop and embed an enabling approach.
* Conduct reviews and benchmarking of individual outcomes and care levels.
* Shape the role by creating standard operating procedures to embed the practice across teams.
* Travel across the county as required to meet individuals from diverse backgrounds.
* Support the flexible working arrangement, balancing home and office duties, and receive management and peer support.
Qualifications and Experience
* Qualified Social Worker with Social Work England registration.
* Some management experience.
* Ability to work from a variety of locations and travel countywide independently.
* Strong communication skills, able to convey difficult decisions to colleagues and other professionals.
* Autonomous and proactive, confident in making decisions independently and seeking guidance when appropriate.
