Overview

Warwickshire County Council is seeking a Social Care Practitioner for our county‑wide Reablement Service. You will assess customers aged 18 and over in their own homes and deliver a therapy‑led care package as part of a multi‑professional team.


Responsibilities

* Assess customers aged 18 and over in their own homes, addressing functional difficulties such as cognitive, physical, environmental and psychological issues.
* Plan and deliver a therapy‑led care package in partnership with Social Care Practitioners, Occupational Therapists, Reablement Officers and Assistants.
* Support the Reablement Service across the county, based in the South of Warwickshire, and provide coverage during busy periods and on Hospital teams when needed.
* Assist the Moving on Bed (MOB) service for residential and Extra Care Housing settings, facilitating hospital discharges for up to six weeks.
* Maintain accurate and up‑to‑date records, comply with safeguarding procedures and pass relevant pre‑engagement checks (including a DBS check).
* Ensure accurate spoken English to meet the Fluency Duty requirement and provide clear advice to the public.


Working hours and location

Monday to Friday, plus one Saturday every seven weeks. Bank holidays are worked on a rota basis. Base location is South Warwickshire with travel across the county.


Qualifications

* Relevant qualification or experience in social care or a related health field.
* Current DBS and other safeguarding checks.
* Good communication and computer skills.
* Desire to work collaboratively in a multi‑professional team and commitment to customer choice and control.
