Physical Health Support Worker Lead

We are looking for an experienced Physical Health Support Worker Lead to join our team at Cygnet Hospital Ealing, who is experienced in working within general hospital outpatient clinics.
Hours: 40 hrs a week (Monday - Friday, 9am - 5pm)
Responsibilities
* Embed physical health care policy and promotion, supporting a GP clinic, Clozapine clinic and other physical health interventions within the hospital.
* Support and assist patients with their individual physical health care plans and input into these.
* Perform basic health checks, including monitoring patients, taking blood, ECG, blood pressure, respiratory rate, side effect monitoring, medication management.
* Coordinate a GP clinic and lead a health awareness session with staff.
* Establish, lead and co‑coordinate evidence‑based physical health practice initiatives across the service and coordinate ongoing monitoring of physical health of all patients including annual health checks in close collaboration with ward doctors and physical health nurses.
* Lead on infection control practice for the hospital, including national clinical standards.
* Provide a programme of health promotion and education in close collaboration with the dietician and ward physical health lead nurses.
* Coordinate the implementation of physical health screening across the service, including access to cervical screening for women.
* Work closely with ward doctors and physical health leads to ensure access to primary care services including dental and eye checks.
* Maintain working knowledge of legislation, company policies and relevant local & national guidance on physical health guidance within mental health services.
* Contribute to national audits and conduct clinical audits to ensure standards are maintained in line with company policy and national best practice.
* Ensure accurate documentation in MyPath and other data records.
* Ensure the service has required medical equipment and local protocols for safe use of medical devices, educate staff, and conduct audits.
Qualifications
* Maths and English GCSE or equivalent.
* Further qualifications in Health and Social Care or Leadership.
* Confident in undertaking physical health assessments including venepuncture / phlebotomy, ECG, blood pressure, respiratory rate, side effect monitoring, medication management.
* Working currently in a healthcare setting, preferably in a mental health setting.
* Evidence of continued professional learning and willingness to train and develop new skills.
* Trained in phlebotomy and other areas of physical health.
* Infection control lead training or willingness to undertake an IPC course.
* A commitment to patient recovery to effectively monitor, manage and reduce risk.
