About HEART (Home Environment Assessment And Response Team)

HEART is a partnership service across the five district councils in Warwickshire. Our Occupational Therapists and Housing Assessment Officers work together to assess customers’ needs and housing conditions, providing tailored solutions such as equipment, minor adaptations, and Disabled Facilities Grants (DFGs) for major adaptations. Our aim is to enable people to live independently by ensuring their homes are safe, accessible, and suitable for their needs.


About The Role

This role is based in Nuneaton, with flexible and agile working arrangements, including home working. Attendance in the office is required for induction, team meetings, and when service needs require. While much of your caseload will be focused on North Warwickshire, Rugby, and Nuneaton & Bedworth, this is a county‑wide role, and you will be expected to work flexibly across all areas of Warwickshire in response to service demand. You will manage a varied caseload, carrying out assessments and working collaboratively to plan and deliver adaptations that enable people to remain safe and independent at home. This is a rewarding and sometimes complex role requiring confident decision‑making, adaptability, and a proactive approach.


What You Will Be Doing

* Managing a caseload from initial assessment through to completed adaptations
* Carrying out holistic, person‑centred assessments
* Recommending equipment, minor adaptations, and major works via DFGs
* Working closely with technical officers, contractors, and architects
* Collaborating with colleagues across health, housing, and social care
* Ensuring a seamless and high‑quality customer experience

You will be supported through regular clinical supervision and benefit from a strong team commitment to continuing professional development.


What We Are Looking For

* A Diploma or Degree in Occupational Therapy
* Current HCPC registration
* An interest in housing adaptations and Disabled Facilities Grants
* Strong problem‑solving skills and the ability to think creatively
* Confidence managing a varied and sometimes complex caseload
* The ability to work independently and make sound professional decisions
* Excellent communication and partnership‑working skills
* Confidence in using IT systems

You will need to be comfortable navigating multiple IT systems, quickly learn new platforms and processes, and be willing to travel and work across the county as required.

The Fluency Duty is outlined in the Code of Practice on the English language requirements for public sector workers. The Fluency Duty for this role is Required. You must be able to speak and provide advice in accurate spoken English to the public.
