Responsibilities:
- Lead holistic, person-centered assessments that identify individual strengths and personal outcomes rather than just service deficits.
- Apply National Eligibility Criteria to ensure resources are directed to those with substantial or critical needs.
- Co-produce robust support plans that transition individuals from acute or crisis settings into long-term community stability.
- Expertly guide service users and their families through the 4 SDS Options, ensuring they understand their rights and responsibilities.
