Senior Practitioner Referral and Assessment Hub
We are continuing to develop our referral and assessment hub and are looking to recruit a B7 Senior Practitioner to support referrals, assessments and clinical leadership within the hub.
Referrals will be triaged by a member of the team, and a decision will be made on the onward journey for that service user.
You may provide assessments for the team and will complete those assessments at that designated base or at a venue agreed with the service user and carer.
This post is a full time position (37.5 hours per week) and the working hours will be 9am - 5pm 5 days per week. The post is based at Claire House Surgery although the post holder will be expected to travel across the Borough to attend satellite clinics.
This post includes participation in the Bronze on call rota.
Main duties of the job
Key duties will include:
* Triage all referrals coming into the referral and assessment hub as part of a team, providing an onward journey if not appropriate for Community Services.
* Completing assessments for those people requiring specialist mental health services under Community Mental Health Team.
* Complete assessment for those inpatients who may require a secondary service upon discharge.
* Provide line management where required.
* Link in with your MDT and attend their weekly meeting.
* Develop and maintain relationships and pathways with other services.
* Promote awareness of the role of the referral and assessment hub within the local service area/team, GP practices and the wider community.
* Support service management in conjunction with the team manager.
* Work with the Team Managers to support policy and service improvement and development.
* Support service user, carer and stakeholder involvement.
* Promote a culture of learning, education and professional development.
* Champion quality assurance by establishing quality standards relating to the service user and carer experience, implementing Divisional Action Plans and undertaking complaint investigations.
* Adhere to the principles of safeguarding of children and vulnerable adults in line with Trust Policy and local reporting procedure and ensure junior staff compliance with this.

* Ability to work over 24 hour, 7 days a week team, to facilitate line management and support practitioners.

Knowledge
* Offer advice and consultation to members of the multi-disciplinary team based on solid clinical experience of mental health nursing.
* Support the Team Managers and other Senior Practitioners / Professional Leads in the daily operation of the Teams.
* Lead on the implementation of the Trust-wide Strategies across the Directorate.
* Facilitate a culture of lifelong learning, including involvement in the process of Individual Development and Personal Review (IDPR), and Personal Development Plans.
* Participate in and ensure provision of Clinical Supervision, both as Supervisee and Supervisor, and an understanding of the Knowledge, Skills Framework under Agenda for Change.
* Promote a learning environment for students.
* Ensure compliance with policy, procedure, and protocol, in consultation with the Team Managers, other Senior Practitioners and Professional Leads.
* Participate in multi-agency meetings, e.g., Protection of Vulnerable Adults, Child Protection.
* Understand the Clinical Governance agenda and provide professional representation in local groups supporting consultation and implementation.
* Full understanding of the Care Programme Approach including the needs and rights of Carers.
* Understanding of current national policy and initiatives in relation to severe and enduring mental health issues.
* Good knowledge of assessment and triage of referrals.
Experience
* Post registration experience of working in multi-disciplinary Home Treatment team or other mental health community services.
* Evidence of post registration specialist training and development.
* Experience of participating in audit and an understanding of implementing Action plans relating to the outcome of audits.
* Knowledge of requirements under the Care Programme Approach including the needs of carers and the mental health Minimum Data Set.
* Previous experience in a clinical leadership role.
Qualifications/experience
* Professional qualification and current registration (Qualified RMN, OT or Social Worker).
* Post registration training relevant to mental health/leadership/management.
