About the Role : As a Social Worker, you will provide essential support to individuals and families facing a wide range of challenges. You'll assess needs, develop care plans, and work alongside multidisciplinary teams to safeguard vulnerable people and improve their quality of life.

Key responsibilities include:
* Comprehensive Assessment: Conduct detailed assessments and risk evaluations for service users.
* Care Planning: Develop, implement, and review tailored support plans. * Safeguarding: Ensure the protection of vulnerable children and adults.
* Advocacy: Support clients to access healthcare, housing, benefits, and community resources.
* Collaborative Working: Partner with healthcare professionals, local authorities, and voluntary organisations.
* Documentation: Maintain accurate and timely case notes in compliance with professional standards.
Who We Are Looking For:
* Education: Degree in Social Work (or equivalent).
