Vacancy Job Title: Case Manager (Part Delivery)
Vacancy Location: Covering Northumberland, Tyne & Wear areas, with hybrid working and session work included.
Salary / Rate of Pay: FTE £27,000
Contract type: Full Time 37 hours, Permanent
Shift pattern(s) available: Hybrid working, office days are Tuesday and Thursday (9am to 5pm) for case management and administrative tasks. Flexibility is essential, as session delivery (18hrs per week) will be required based on service needs and caseload, which may be outside of the typical working pattern.
* A degree in Psychology, Occupational Therapy, Nursing, NVQ Level 3 in Health and Social Care or related field or, demonstrable work experience with vulnerable adults is essential.
* Experience in providing support to individuals with Brain Injuries, following Rehabilitation care plans, and understanding their unique needs and challenges.
* Proficiency in performing personal care tasks, ensuring the well-being and comfort of the individual under your care.
* A naturally positive attitude that fosters a supportive and uplifting environment.
* Proven ability to inspire and motivate the individual you support, empowering them to pursue their goals and aspirations.
* Full UK driving licence and access to a vehicle.

Support Provision:
* Your working hours will include support delivery in clients' homes and office-based work.
* Deliver high quality, safe, and person-centred support.
* Participate in contingency planning for absences and holiday cover.
* Safeguard and act in the best interests of the person supported.
* Develop rapport and a professional supportive relationship with clients and their families.
* Identify achievable support and rehabilitation goals with clients.
* Manage and lead Support Workers to follow care plans.
* Coordinate support sessions with stakeholders, ensuring funded hours are delivered appropriately.
* Keep the Service Manager and Operations Manager informed of support delivery, achievements, and issues.
Line Management:
* Act as line manager to Support Workers.
* Conduct regular supervisions and observations.
* Provide support, guidance, and training to staff.
* Lead by example and communicate effectively.
* Assist in recruiting Support Workers based on individual needs.
* Brief and train Support Workers on specific client needs.
