The post holder will work as a key member of the Living Well Team, delivering the Short Term Offer (STO) to individuals experiencing mental health difficulties. The role focuses on providing timely, person-centred, recovery-oriented interventions to support individuals to stabilise, build resilience, and access appropriate ongoing support where required.
The practitioner will undertake holistic assessments, develop collaborative care plans, and deliver brief therapeutic interventions aimed at promoting independence, reducing risk, and preventing escalation or crisis.
Working within a multidisciplinary team, the post holder will contribute to a flexible, responsive service that promotes early intervention, maximises service user choice, and strengthens links with wider community resources.
Main duties of the job
1. Undertake comprehensive, person-centred mental health assessments, including risk assessment and management.
2. Deliver evidence-based, short-term interventions in line with STO principles psychoeducation, brief therapeutic approaches, signposting).
3. Develop and review individualised care and safety plans collaboratively with service users and their networks.
4. Promote recovery, self-management, and independence, supporting individuals to achieve their personal goals.
5. Work in partnership with carers, families, and other professionals to ensure coordinated care.
6. Facilitate timely access to appropriate services, including stepping up or stepping down care as required.
7. Maintain accurate, timely clinical records in line with organisational and professional standards.
8. Contribute to multidisciplinary team discussions, case reviews, and service development.
9. Identify safeguarding concerns and act in accordance with local and national policies.

You will be clinically responsible for supporting the delivery of a flexible, responsive local service, taking forward an Integrated Wellbeing Pathway in the designated locality, which effectively supports the delivery of the Community Mental Health Framework. The post holder will work collaboratively with colleagues, key partners, and those within the local community to develop services to meet the mental health needs of the community.
You will employ a wide range of skills, building on and developing their expertise and actively seeking advice and collaboration with issues which may not be readily within their sphere of practice. The requirements for this post are to remain within the guidelines for professional practice inclusive of HCPC/NMC requirements or equivalent professional body.
The role will involve undertaking comprehensive person-centred psychosocial assessments (conversations), risk assessments and formulation of presenting mental health needs. The practitioner will build therapeutic relationships, develop a plan of care with that person and assist them through their difficult time by empowering and enabling them to take control over their health.
The role involves supporting and motivating individuals to improve their mental health by helping them develop self-care skills, build resilience, and better utilise community resources. A key part of this developing role will be to make and maintain relationships with community partners including statutory organisations, Voluntary, Community and Social Enterprise (VCSE) organisations and the local communities who we serve. Hearing and responding to the voice of lived experience is a key element for developing this service.

Qualifications
Essential criteria
21. Current registration with Nursing & Midwifery Council under, Registered Professional with HCPC for Occupational Therapy, or Social Work England for Social Work
Desirable criteria
22. Relevant formal intervention skills
23. Relevant training in personality disorder withing with complex needs and/or trauma informed practice
Experience/Knowledge
Essential criteria
24. Post qualification experience in the relevant field / Compteted preceptorship or equivilent
25. Experience of community working
26. Able to meet the travel requirements of the post
27. Good verbal/ non verbal communication skills
28. Good IT skills
29. Able to work independently, as well as part of a team
30. Extensive clinical experience including individual and group work.
Desirable criteria
31. Experience of linking with partner agencies
32. Understanding of the living well programme
33. Good time management
