Detailed job description and main responsibilities

* Accurately add encounters and referrals for patients requiring secondary care.
* Communicate effectively with patients who may have difficulty expressing themselves, using de-escalation techniques when necessary.
* Provide VBT (Very Brief Advice) to current smokers, offering encouragement and support to quit smoking.
* Work collaboratively with multidisciplinary teams, sharing knowledge while respecting role boundaries.
* Line manage and support Patient Contact Assessors (PCAs) to ensure the team meets performance targets.
* Act as the first point of escalation for complex patient queries and team concerns.
* Assist in recruitment, training, and onboarding of new PCAs, ensuring they are well-equipped for their roles.
* Support the Performance and Pathway Manager and the Failsafe and Administration Manager in performance reviews, one-to-one meetings, and staff development.
* Monitor sickness absence and performance within the team, conducting return-to-work interviews and escalating concerns as required.
* Oversee team rotas and workload, adjusting as needed to maintain service delivery.
* Schedule and facilitate monthly team meetings, preparing agendas, ensuring productive discussions, following up on action items, and promoting effective communication and collaboration within the team.
* Approve and monitor annual leave requests, ensuring adequate team coverage, and maintaining effective scheduling to meet service needs.
Secondment will be considered for internal candidates - If you are successful after interview, you must obtain approval from your current line manager to accept the offer.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
