Registered Nurse Learning Disability or Registered Mental Health Nurse required for NHS Coventry & Warwickshire Integrated Care Board, West Midlands, England.
Job Summary
The post holder will facilitate multi-disciplinary assessments and patient reviews in line with standard operating policies and procedures. They will adhere to the relevant National Framework and guidance, the NMC Code of Professional Conduct, and work within established policies, procedures, protocols and systems to ensure quality standards and performance as directed by the Complex Case Manager / Clinical Commissioning Manager.
Main duties
* Manage day-to-day clinical care and case management for a caseload of patients funded by NHS Continuing Healthcare (CHC), including those jointly funded with the Local Authority. This includes assessment, review, and ongoing management to ensure safe, effective care and funding alignment with CHC criteria.
* Work autonomously while collaborating with the team to target specific elements of clinical review within the CHC service for Coventry, Rugby and Warwickshire.
* Case manage your own clinical caseload end-to-end, including resolving issues with funding, placements, invoices and complaints.
* Identify and arrange services to meet the needs of individuals who qualify for NHS Continuing Healthcare and for the healthcare element of joint care packages as appropriate.
* Undertake investigations into an individual’s eligibility for NHS Continuing Healthcare following challenges or disputes.
* Comply with the Data Protection Act 1998 and Code of Practice on confidentiality and data protection.
* Participate in ongoing professional development and clinical supervision in line with NMC guidelines.
* Ensure compliance with statutory and mandatory training; demonstrate flexibility, reliability, motivation, and ability to prioritise workload; work under pressure and meet targets and deadlines.
* Demonstrate the ability to resolve problems and communicate effectively with CHC colleagues, social services, care home providers, hospital discharge teams and other healthcare professionals and stakeholders.
* Have good IT skills; an understanding of working with care homes is helpful but not essential.
Qualifications
Essential
* Registered Nurse Assessor Role - Registered Nurse Learning Disability or Registered Mental Health Nurse
* Extensive post-qualification experience in Learning Disability or Mental Health Nursing
* Ability to travel (car owner)
Desirable
* CHC working experience
* Experience of writing a Complex Clinical Report and meeting work deadlines
