Primary Care Nurse – HMP Garth
Practice Plus Group is recruiting a Primary Care Nurse to join our Primary Care Team at HMP Garth, a high‑security, category B prison in Lancashire, housing approximately 792 male inmates.
Overview
The role is full‑time (37.5 hours per week) and involves day, night and weekend service across an 8‑week rolling rota. The primary objective is to deliver a nurse‑led, multidisciplinary service that prevents hospital admissions and ensures high‑quality, patient‑centred care within a secure environment.
Responsibilities
* Work within a multi‑disciplinary team to provide a nurse‑led service and prevent unnecessary hospital admissions.
* Undertake medication rounds, dispense controlled medication, and attend emergency response situations.
* Assess patients and conduct reception screening.
* Deliver nurse‑led clinics to designated patient groups with support from the multidisciplinary team.
* Provide evidence‑based care, actively seek and respond to patient feedback, and personalise care accordingly.
* Deliver non‑judgemental care that makes a tangible difference to patients.
* Forge strong links with external partners and stakeholders to promote best practice, clinical supervision, and continuous quality improvement.
* Share knowledge, skills and expertise to strengthen safe, effective multi‑disciplinary teamwork.
Qualifications
* Registered Nurse with the NMC (Adult Nursing).
* Evidence of continued professional development.
* Understanding of the complexities of multi‑disciplinary and multi‑agency working.
* Non‑judgemental, compassionate approach to care.
* Excellent communication skills.
Although not essential, experience in a hospital, community or prison environment is advantageous.
Hours & Schedule
* Monday – Sunday service, days and nights.
* 37.5 hours per week.
* 8‑week rolling rota:
o Weeks 1, 3, 5 – Monday, Tuesday, Saturday, Sunday, 07:30‑19:45 (days).
o Weeks 2, 4, 6 – Wednesday, Thursday, Friday, 07:30‑19:45 (days).
o Week 7 – Monday, Tuesday, Saturday, Sunday, 19:30‑07:45 (nights).
o Week 8 – Wednesday, Thursday, Friday, 19:30‑07:45 (nights).
Salary
Annual salary ranges from £34,428 to £41,659 per annum, depending on experience and qualifications.
