We are seeking a compassionate and experienced Social Worker to join our multidisciplinary team at Marie Curie Hospice Edinburgh. This vital role involves delivering a comprehensive social work and bereavement service across the hospice and, where appropriate, in the community. You will work closely with patients, families, and carers—supporting them through pre-bereavement and end-of-life care—while liaising with local councils and external organisations to coordinate services.
The role includes conducting 1:1 sessions, attending bed meetings, managing referrals, and overseeing admissions and discharges. You'll also participate in discharge planning meetings and multidisciplinary team sessions, advocating for patients and families to ensure their needs are met. As an autonomous practitioner and collaborative team member, you'll uphold national standards and the social work Code of Conduct, contributing to the delivery of compassionate, person-centred care.
What you will be doing:
* Deliver personalised social work services to both patients and their loved ones.
* Conduct in-depth assessments to identify and address the unique needs of each patient and family member.
* Provide bereavement support and ongoing care to individuals receiving hospice services, extending to the wider community when needed.
What we're looking for:
* Social Worker will also be accountable for ensuring own practice meets minimum national standards and is compliant with the social work Code of Conduct
* Post-qualifying experience supporting patients and families affected by life-limiting or terminal illness
* Proven ability to work effectively within a multidisciplinary team, contributing to holistic care planning
* A recognised degree in Social Work or equivalent qualification
* Current registration with the relevant professional body (e.g. Care Council)
* Candidate must be able to evidence appropriate experience of working with those who have complex care needs and/ or life limiting illness.
* Ability to work independently as an autonomous practitioner, while also being a collaborative team player
