Essential criteria
* Willing and able to apply existing experience to community settings and gain necessary skills to work effectively in the community.
* Willing to professionally develop and undertake further training as identified, including Trust Band 3 Extended Skills programme and Care Certificate.
* Excellent communication skills.
* Excellent interpersonal skills.
Job Specific Experience
Essential criteria
* Desire to work in the primary care setting.
* Ability to work alone.
* Good IT skills.
Desirable criteria
* Community/Acute Hospital experience.
Business Travel

* Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business.
