About the Role: As a SEND Keyworker, you’ll be at the heart of delivering statutory assessments and managing Education, Health and Care Plans (EHCPs) for children and young people across the borough. This is a dynamic and varied role where you will work closely with families, schools, colleges, and multi-agency partners to ensure the best outcomes for children and young people with SEND. Key Responsibilities: Coordinate outcome-focused, person-centred multi-agency assessments and support planning. Act as the named officer and first point of contact for allocated children, families, and settings. Attend and chair co-production meetings, evaluating reports and making evidence-based recommendations. Support families in navigating SEND processes, including attending mediation or tribunal as needed. Monitor and review progress against EHCP outcomes, advising on provision and use of personal budgets.
