Early Intervention in Psychosis (EIP) Care Coordinator (Band 6) for Nottinghamshire Healthcare NHS Foundation Trust.
Responsibilities
* Support and manage a caseload of service users experiencing first episode psychosis.
* Screen and assess service users referred to the EIP team.
* Participate in clinical decision making within the multidisciplinary team to determine appropriate management plans in collaboration with service users and their families.
* Use standard outcome measures (HoNOS, QPR, DIALOG) to monitor recovery.
* Travel across Nottinghamshire to support service users, their families and carers.
* Utilise digital technologies to support service delivery.
Qualifications
* Current registration as a Mental Health Nurse, Occupational Therapist, or Social Worker (or equivalent).
* Previous experience in a community health setting and working with patients with psychosis.
* Experience or knowledge of Early Intervention in Psychosis team interventions.
* Knowledge of challenges faced by individuals with psychosis and at‑risk mental states.
* Full UK driving licence and access to a vehicle.
* Experience of caring responsibilities (required).
* DBS check required.
