Position Details
Primary Care Coordinator – Walk‑in Service – Ospadal Uibhist agus Bharraigh, Benbecula, Band 3, £29,103 – £31,409 pro rata. Distant Islands Allowance, £1,461 pro rata. Full‑time and part‑time hours available. Fixed‑term post – 31 March 2024.
Role Overview
A unique opportunity for a dynamic individual to work with our innovative and evolving Walk‑in Service, designed to relieve pressure on local GP practices across the Western Isles, improve patient access and enhance resilience of Primary and Urgent Care services.
Responsibilities
* Coordinate access via nine GP practices and walk‑in patients, supporting the clinical team to deliver a consistently high standard of patient care.
* Develop and maintain effective working relationships with GP Practice Representatives and Primary Care colleagues to ensure the patients receive appropriate support throughout their service journey.
* Manage same‑day urgent care for temporary residents and visitors across the Western Isles, and arrange telephone or Near‑Me (video conference) appointments as necessary.
* Collaborate with the Primary Care Coordinators, Advance Clinical Practitioner and GP.
Requirements
* Registered with the PVG Scheme (Protecting Vulnerable Groups Scheme).
* Right to work in the United Kingdom.
* Post is not eligible for relocation expenses.
