Integrated Care Clinician (Nurse) - Swansea
Overview
The post holder will undertake telephone triage assessments of patients in an efficient way and advise the most appropriate intervention to meet patient needs, using best clinical practice and adhering to Trust policy, protocols and guidelines. The role is autonomous, assessing and advising callers within agreed guidelines as part of NHS 111 Wales’ urgent care service.
Responsibilities
* Perform telephone triage assessments of patients in a timely and clinically appropriate manner.
* Advise on the most suitable intervention to meet patient needs using clinical judgment and guidelines.
* Work autonomously within agreed guidelines, supported by senior clinicians and a multidisciplinary team.
* Collaborate with urgent care GPs, Pharmacists, Mental Health Practitioners and other colleagues as part of a multi-disciplinary team.
* Contribute to a remote clinical decision‑making environment to deliver patient‑centred care.
Qualifications
* NMC Registered Nurse (adult or child) or HCPC Registered Paramedic with demonstrable post‑registration experience in front line services.
* Evidence of regular update training and ongoing professional development as required by the regulatory body.
Experience
* Strong listening and communication skills.
* Demonstrated ability to work in a team and also use own initiative to make decisions.
* Sound clinical decision‑making capabilities and up‑to‑date clinical knowledge; computer literate and committed to personal development.
* Desirable: ability to speak Welsh.
Aptitude and Abilities
* Commitment to ongoing training and mandatory updates.
* Good self‑management, punctuality and reliability.
* Ability to work under own initiative without direct supervision.
* Flexible approach to work, including rotating shifts, nights and weekends.

* Salary: £39,263 to £47,280 per annum (pro rata where applicable)
