Occupational Therapist – Adults Services Location: Monmouthshire (based at Chepstow Community Hospital) Pay Rate: £32–£36 per hour Working Hours: 9:00am – 5:00pm, Monday to Friday Working Pattern: Community-based with travel to service users’ homes Contract Type: Temporary About the Role: An excellent opportunity has become available for an experienced Occupational Therapist to join the Adults Community Team in Monmouthshire. This role focuses on delivering high-quality, person-centred Occupational Therapy interventions to help individuals maintain independence, avoid unnecessary hospital admissions, and access the right care, equipment, and home adaptations. Key Responsibilities: Deliver Occupational Therapy interventions within service users’ homes and community settings Complete comprehensive assessments and develop tailored intervention plans for individuals with long-term or complex needs Support admission avoidance through timely and effective therapeutic input Recommend and arrange assistive equipment and home adaptations, including Disabled Facilities Grants (DFGs) Conduct housing assessments and contribute to manual handling and reablement plans Participate in a weekly duty rota, responding to queries and urgent referrals Act as a Trusted Assessor to set up and review care packages Manage a varied caseload efficiently, demonstrating excellent organisational and clinical reasoning skills Key Relationships: You will work closely with: Community-based service users and their families/carers Multidisciplinary teams within Chepstow Community Hospital Social care teams, housing departments, and external agencies Duty team colleagues and wider Occupational Therapy services Care providers and equipment suppliers Requirements: Qualified Occupational Therapist Registered with the Health and Care Professions Council (HCPC) Experience working within community or hospital-based adult services Strong assessment and communication skills Flexible, motivated, and able to work independently and as part of a team Welsh language skills are desirable but not essential Why work through Pertemps?
