Job Summary

Are you passionate about making a difference to the lives of older adults? We are looking for an enthusiastic and motivated Community Mental Health Support Worker to join our dynamic Community Mental Health Team. In this role, you'll provide high‑quality, compassionate care to adults with complex mental health conditions. You'll work collaboratively with a well‑established team to deliver personalised treatment plans and support patients and carers as they navigate complex mental health challenges. The role may also involve occasional work with older adults to support the wider goals of the service.

Main duties

Clinical Support Workers are responsible for the delivery of care and support for service users within the community mental health setting and for reporting on the service users’ wellbeing under the guidance of a registered mental health practitioner.

You will also be responsible for delivering a range of therapeutic interventions and other aspects of the service user’s care plan under the guidance of a registered mental health practitioner.

You will liaise with all agencies necessary to the health and wellbeing of the service user and their carers.

Having access to transport is essential as we are a community‑based service offering support in patients’ homes and local communities.

Person Specification

Essential
* NVQ Level 3 or equivalent knowledge and experience
* Basic risk and mental health assessment
* Record keeping
* Experience of mental health difficulties – either lived experience or as a lay or professional carer
Desirable
* Basic clinical skills: venepuncture, ECG recording, clinical observations
* Delivery of brief interventions for healthy lifestyle promotion: smoking cessation, alcohol reduction, healthy eating, activity and exercise
* Experience of working within a community care setting
* Experience of working with small groups
