We are looking for a District Nurse / Community Sister / Charge Nurse to join our multidisciplinary community team. This role involves coordinating and delegating patient care across secondary and social care settings. You will be responsible for managing a caseload, providing clinical case management for patients with complex needs at risk of deterioration, which could lead to declining quality of life, avoidable hospital admissions, or unnecessary hospital stays.

Some main duties include (but are not limited to):
1. Managing a patient caseload and delegating responsibilities within the multidisciplinary team.
2. Applying advanced assessments of physical, mental, and social needs to provide clinical care and health promotion interventions.
3. Interpreting clinical signs, symptoms, and diagnostic tests to develop treatment plans in collaboration with medical practitioners.
4. Enabling patients to maintain independence through self-management strategies.
5. Developing and analyzing audits, interpreting data, and using findings to improve clinical practice.

Some key responsibilities include:
* Managing a designated patient caseload and overseeing care delivery by the team.
* Supporting the team leader and clinical lead in quality improvement, risk management, and ensuring compliance with CQC and clinical audit standards.
* Overseeing clinical areas and facilitating junior staff leadership.
* Deputising for the Clinical Nurse Lead when necessary, assisting in recruitment, appraisals, risk assessments, and performance management.
* Providing clinical supervision, peer review, leadership, and mentorship to junior staff.
* Managing resources, equipment, and supplies efficiently, considering the trust’s financial position.
* Participating in personal development planning and acting as a practice assessor for staff and students.
