The ISW role involves supporting patients to wash and dress, promoting independence, encouraging and motivating patients with managing their nutrition, and medicines management from an administration chart under supervision. The team works from 8am-10pm, 7 days a week, with flexible shifts, including full and part-time opportunities. The role involves independent travel between patients’ homes and the office base in a timely manner.
The ISW role provides outstanding care and support to patients in their own homes, working alongside Senior ISWs, Nurses, and Therapists to optimize patient outcomes. Responsibilities include personal care, medication support, and assisting with eating and drinking, with opportunities to develop further competencies supporting the multidisciplinary team.
The Urgent Community Response team works alongside the integrated night service, which provides overnight cover.
