Bank Band 6, CPN/Social Worker, Hemel - SW1367

Closing date: 01 December 2025.

We are seeking an experienced Community MH Nurse/Social Worker to join our Bank Bureau within the Hertfordshire Partnership University NHS Foundation Trust on a Bank Contract, based in Hemel.

The post-holder will be responsible for the clinical and operational day‑to‑day service delivery, ensuring that an individualised, comprehensive assessment and Care Management service is effectively delivered.


Main duties of the job

Job Summary: Responsibilities include the clinical and operational supervision of junior nursing staff and students as delegated by the team manager. The post-holder will be accountable for delivering quality services that are effective and efficient in line with the organisation's objectives and performance indicators. Contribute to strategic service development working collaboratively with other agencies.

Key Relationships: Support the Team Leader in establishing and maintaining effective inter‑team and inter‑agency communication. Provide effective liaison throughout the care pathway to service users, their carers and others, responding sensitively to all needs especially in highly emotive settings. Maintain joint working with others, developing positive working relationships and clear communications with MDT and other professionals.

Duties and Responsibilities

Clinical Responsibilities:

* Undertake assessment of need for service users and their families who meet the service eligibility criteria.
* Formulate a care plan and risk assessment, review its effectiveness and re‑plan using evidence‑based interventions in partnership with the service user.
* Identify and respond to acute presentations to manage risk and promote safety and well‑being.
* Utilise clinical judgement to evaluate service users' physical and psychological responses to treatment.
* Initiate, plan and deliver effective risk management systems within the clinical area.
* Respond consistently and timely to safeguarding cases in line with departmental procedures.
* Provide a responsive effective service to individuals in crisis.
* Plan, organise and prioritise resources and tasks daily to meet service and organisational needs.
* Take a leading role in the day‑to‑day management of the service, supporting the Team Leader, including recruitment, retention, rostering and attendance management as required.
* Work with colleagues and partners within the organisation.
* Plan and deliver treatment interventions in line with best practice and local and national guidelines.
* Act as a Care Coordinator ensuring high‑quality care delivery at all times.
* Take a leading role in designated projects/service development as allocated.
* Support the line manager/service line lead in the effective and timely management of complaints and serious untoward incidents.
* Identify the training needs of junior staff and highlight this to the Team Leader.
* Plan, develop and deliver training to others as needed.
* Undertake Approved Mental Health Professional (AMHP) duties or be willing to undertake AMHP training.

Physical & Emotional Working Conditions and Environment:

* Document service user care by recording information on electronic care record and demonstrate keyboard skills.
* Sit with service users for extended periods, often within pressurised circumstances, sustaining frequent, intense concentration.
* Adapt to the physical environment, particularly when meeting service users in their homes, and adhere to Trust Health & Safety policies.
* Coping with frequent travel across the Trust.
* Drive own vehicle between bases.
* Involved in physical procedures including administration of oral and injectable medications, storage, transportation and monitoring of physical responses.
* Travel between appointments across a wide geographical area, managing numerous visits daily across the west and south of the county and occasionally further.
* Share office space in a hot‑desk arrangement and work in an open‑plan environment; visits to service users occur mainly in their own homes and can include a range of environments.
* Work frequently in distressing, challenging and emotionally demanding situations requiring mental effort.
* Potential exposure to unpleasant and distressing working conditions; some service users may exhibit aggressive and abusive behaviour.


Person Specification


Qualifications

* Training and experience in safeguarding.


Previous Experience

* Experience working in a range of settings relevant to the post.
* Experience undertaking assessments focused on personalisation, choice and control, and care planning.
* Experience and knowledge of safeguarding processes.
* Experience working with acute mental health problems in a community setting.


Physical Skills

* Ability to visit and attend meetings at a variety of locations and environments.
* Full valid driving licence and access to a car for business use (unless a disability as defined by the Equality Act 2010).
