Working alongside physiotherapy‑informed practice and Social Work approaches, our team delivers high‑quality, Care Act compliant assessments and develops personalised reablement and support plans. Our aim is simple:- To empower people to live meaningful, fulfilling lives as they transition back to their own home or into community living. The post holder will: Deliver holistic, strength based reablement for adults with complex needs within a residential reablement setting. Promote and improve independence for adults with complex needs by provision of equipment and adaptations, moving with dignity, and interventions.
