💷 Pay: Up to £33.00/hour (Umbrella)
Are you a compassionate and experienced Adult Social Worker looking to make a real difference in people's lives? Join our team supporting hospital discharge and community care pathways!
Your Role:
🏥 Work within the SPA - triage for hospital discharges
📋 Apply eligibility criteria under the Care Act
✅ Complete Continuing Health Care Assessments / Reviews
🧠 Conduct Mental Capacity assessments in line with MCA 2005
🏡 Use your knowledge of hospital discharge pathways to support smooth transitions
What We're Looking For:
Experience in adult social care or hospital discharge pathways
Strong knowledge of Care Act eligibility criteria and CHC processes
Ability to complete Mental Capacity Assessments confidently
Excellent communication and decision-making skills
