The world of General Practice is busy and ever-changing, so an ability to learn quickly and adapt to rapid change is essential. The role requires the ability to organise and prioritise your workload, and you should be comfortable working independently and as part of a multidisciplinary team.
Main duties of the job
* Deal with incoming queries directly from patients
* Assist with signposting patients to appropriate community or local services, alongside social prescribers
* Act on communications from hospitals or community providers
* Pro-actively ensure patients are booked for chronic disease management appointments
* Assist with ensuring patients participate in screening programmes
* Promote health education, such as participation in self-care week
* Encourage vaccination opportunities like flu, shingles, or childhood immunisations
* Refer patients to in-house social prescribers
* Organise and prioritise own workload

PURPOSE
To provide a practice point of contact for patients in coordinating their care
To ensure timely follow-up and action for patients from communications from community or secondary care
To deal with incoming queries from patients and other healthcare providers
To organise in-practice appointments, follow-ups, or other actions to help the PCN practices provide high-quality, compassionate care to our patient population
Specific job responsibilities
* Deal with incoming queries directly from patients
* Assist with signposting patients to appropriate community or local services, alongside social prescribers
* Act on communications from hospitals or community providers
* Book follow-up appointments, tests, or reviews
* Pro-actively ensure patients are booked for chronic disease management appointments
* Assist with ensuring patients participate in screening programmes
* Promote health education, such as participation in self-care week
* Encourage vaccination opportunities like flu, shingles, or childhood immunisations
* Refer patients to in-house social prescribers
* Participate in practice MDT meetings and organise actions or follow-ups
* Recognise and respond appropriately to sudden deterioration or emergency situations, alerting the team for rapid response
* Organise and prioritise own workload
* Monitor tasks to ensure completion and care delivery through regular audits
* Coordinate care with multi-agencies
* Support patients and carers, collecting feedback on their experiences
* Collaborate with other care coordinators across the PCN to share best practices
* Support practices in delivering on QOF, KPIs, and locally commissioned services
* Maintain accurate patient medication records
* Develop relationships with community, hospital, and wider health systems
* Participate in practice meetings, patient groups, and other engagement activities

Qualifications
* GCSE maths and English or equivalent
* Commitment to professional and personal development
Experience
* Experience working with patients
* Experience in partnership working and building relationships across organisations
* Experience in general practice or NHS/social care settings
