* Salary: £42,839 – £44,075 per annum
* Hours: 37 per week
* Contract: Permanent
Responsibilities
* Manage a complex, high-risk caseload.
* Assess needs and design person-centred care plans.
* Deliver empowering outcomes and champion a re-ablement approach.
* Ensure safety, dignity and independence for individuals.
* Use professional judgement and innovative, solution-focused thinking to shape services.
* Collaborate with health partners, community organisations and other agencies.
* Provide case supervision and professional guidance to colleagues.
Qualifications & Requirements
