Job Summary
Apply highly specialist knowledge in the pharmaceutical management of surgical patients to develop and implement Trust protocols and guidelines, undertake audits and research, support the development of pharmaceutical services across surgical specialties, and provide specialist advice and training to the multidisciplinary team.
Key Responsibilities
* Perform and oversee monitoring and review of drug therapy for inpatients and outpatients under surgical directorates, ensuring compliance with legislation and Trust policies.
* Conduct accurate medicines reconciliation on admission and discharge, optimise medication therapy, and contribute to the discharge process.
* Provide specialist pharmaceutical advice to consultants, junior staff, GPs, nurses, pharmacy staff and patients/carers, evaluating data and applying professional judgment.
* Collaborate with medical and nursing staff to formulate medicine policies and guidelines that promote rational prescribing, monitor adherence and report on effectiveness.
* Deputise for the lead pharmacist for surgery.
* Maintain good working relationships with ward staff, pharmacy supply services and regional/national colleagues.
* Provide structured feedback on medicines safety, efficacy and expenditure to improve service delivery.

Person Specification – Essential
Qualifications
* Master's Degree in Pharmacy
* GPhC registration
* Clinical diploma or equivalent (or working towards)
* Independent prescriber licence or working towards

* Extensive experience as a clinical pharmacist in an acute hospital setting
* Experience communicating and collaborating within a multidisciplinary environment
