Responsibilities
* Manage specialist caseload and maintain associated records.
* Work closely with renal dietitian team members and the wider adult acute dietetic team to ensure timely dietetic care, prioritise workload and support other members (dietetic assistants and student dietitians).
* Cover inpatient and outpatient areas including haemodialysis units.
* Participate in staff training within the MDT, including nursing, medical colleagues, medical students and nursing assistants.
* Collaborate with catering colleagues, AHPs to deliver quality service to renal patients and service users.
* Provide therapy input to renal patients and liaise with other disciplines and agencies for planning and implementation of patient care.
* Liaise with and work as an integral member of the multidisciplinary team, building and maintaining effective working relationships within the Trust and the wider health economy.
* Liaise with dietitians from other Trusts and other healthcare professionals concerning patient treatments and progress with dietary regimens.
* Educate patients, relatives and carers about their condition and ensure care information and support is available.
* Communicate and process complex and sensitive information to/from patients, carers and multidisciplinary team.
* Plan, implement and evaluate specialist treatment plans for patients with complex needs.
