About the Role
We are looking for a motivated and enthusiastic Practice Nurse to join our friendly and supportive team. The successful candidate will lead and deliver nurse-led chronic disease clinics, with a particular focus on hypertension and respiratory care (asthma and COPD).
We welcome applications from experienced practice nurses and from nurses looking to develop in chronic disease management full training and support will be provided. Depending on qualifications and experience, we would also consider appointing a Treatment Room Nurse with a willingness to train into chronic disease management.
Key Responsibilities
· Lead and undertake chronic disease clinics, including hypertension, asthma, COPD, and general respiratory reviews.
· Provide individualised care planning, lifestyle advice, and self-management support.
· Conduct assessments and monitoring in line with clinical guidelines and local pathways.
· Perform and interpret spirometry (if competent or willing to train).
· Support treatment room services where required (wound care, injections, ECGs, cervical screening depending on skill set).
· Maintain accurate and timely clinical records using practice systems.
· Participate in audits, quality improvement work, and health promotion.
· Work collaboratively with GPs, ANPs, pharmacists, and the wider MDT.
About You
· Is a registered nurse (NMC).
· Has experience in primary care or a genuine interest in developing chronic disease management skills.
· Is competent in or keen to train in hypertension and respiratory care.
· Can work independently, prioritise workload, and make evidence-based decisions (or is committed to developing these skills).
· Communicates well and provides compassionate, patient-centred care.
· Works well as part of a supportive, multidisciplinary team.
Desirable (but not essential training can be provided)
· Experience or formal training in spirometry.
· Respiratory or cardiovascular qualifications.
· Chronic disease management modules or primary care experience.
· Familiarity with NICE best practice and local care pathways.
· Treatment room experience (for applicants applying on that basis).
