Health and Wellbeing Coach (Mental Health)
Here in Penwith Primary CareNetwork (PCN) we have created a coaching service as part of the transformationof mental health care in Cornwall. There has never been a more exciting time towork in mental health services, especially in primary care. We want to hearfrom people who have passion and creativity to help shape and build ourholistic service in Penzance, Hayle and St Ives.
We aim to create a space where weunderstand what matters to you, not whats the matter with you. Our newteam will include a diverse range of skills, to listen and support people whoare struggling with their mental wellbeing. We are in the process of creating awelcoming hub in Penzance & Hayle, with outreach supporting our more ruralareas.
Main duties of the job
As a Health and Wellbeing Coach (Mental Health) You will work as part of a team, coaching patients both on a face to face and virtual basis.
This role will be predominantly hub-based,working across multiple locations with some remote working and outreachclinics in Penwith. You will be a valuable member of the mental health &wellbeing team, as well as being aligned to one or more GP practices in thepatch. A driving licence is essential.
We are looking for candidates who are enrolled in,willing to undertake, undertaking or qualified from Health Coaching trainingwith the training delivered by a training organisation listed by thePersonalised Care Institute.

Job responsibilities
Key responsibilities
Manage and prioritise a caseload, taking anon-judgemental approach based on strong communication and negotiation skills,while considering the whole person when addressing existing issues
Use IT and MDT channels to screen patients, withan aim to identify those who would most benefit from health coaching
Provide personalised support to individuals,their families and cares, to support them to be active participants in theirown healthcare; empowering them to manage their own health and wellbeing andlive independently through:
oCoaching and motivating patients throughmultiple sessions to identify their needs, set goals, and supporting patientsto achieve their personalised health and care plan objectives
oProviding interventions such as self-managementeducation and peer support
oSupporting patients to establish and attaingoals that are important to them
oSupporting personal choice and positive risktaking while ensuring that patients understand the accountability of their ownactions and decisions, thus encouraging the proactive prevention of furtherillness
oWorking in partnership with the socialprescribing service to connect patients to community-based activities whichsupport them to take increased control of their health and wellbeing
oIncreasing patient motivation to self- manageand adopt health behaviours
Work in partnership with patients to supportthem to develop their level of knowledge, skills and confidence enabling themto engage with their health and wellbeing and subsequently supporting them inshared decision-making conversations
Utilise health coaching skills to support anddevelop the knowledge, skills, and confidence to manage their health andwellbeing, whilst increasing their ability to access and utilise communitysupport offers
Explore and support patient access to a personalhealth budget, where appropriate, for their care and support
Refer people to other health professionalswithin the PCN as appropriate
Adheres to a code of ethics and conduct in linewith the NHS England health coaching Implementation and Quality Summary Guide.
Wider responsibilities
Develop collaborative relationships and work inpartnership with health, social care and community and voluntary sectorproviders and MDTs to holistically support patients wider health andwellbeing, public health and contributing to the reduction of healthinequalities
Provide education and specialist expertise toPCN staff, supporting them to improve their skills and understanding ofpersonalised care, behavioural approaches and ensuring consistency in thefollow up of peoples goals with MDT input
Raise awareness within the PCN of shareddecision-making and decision support tools
Work closely in partnership with the socialprescribing link workers, to work alongside people who may need additionalsupport.
Maintain and update care records, including onGP practice systems.
Person Specification
Qualifications
* Enrolled in, willing to undertake, undertaking or qualified from health coaching training, with the training delivered by a training organisation listed by the Personalised Care Institute
Personal Attributes
* Compassionate, trauma-informed approach
* Flexible team player who is equally happy working alone or as part of a team
* Ability to work autonomously and make decisions within the boundaries and scope of the role
* Willingness to undertake mandatory training online or face-to-face, and to undertake further training as the service/need develops
* Driving licence with access to a car
* Enhanced DBS check
Experience
* Experience of working with people who have trouble coping with their emotions and/or have experienced poor mental wellbeing
* Experience of working with people with a wide range of issues, for example drug and alcohol dependence, self-harm, domestic violence
* Basic understanding of a range of prevalent mental illnesses, how and why they may develop and the symptoms someone might experience
* Experience of working with neuro-divergent people
* Understanding of health and social inequalities in parts of Penwith
* Basic understanding of medication, physiological comorbidities, and the effects of these on a persons presentation
* Good knowledge of the voluntary sector and charities that are available locally to support bio/psycho/social wellbeing
