Schedule:
Monday to Friday: 9 AM - 6 PM
Rotating Saturdays: 9 AM - 6 PM

Key Responsibilities
* Evaluate physician medication orders for appropriateness, dosage, potential interactions, and route of administration. Make recommendations for therapy changes as needed.
* Maintain a comprehensive patient/resident database, including current and past medical history, clinical data, and concurrent medications.
* Ensure appropriate laboratory tests for monitoring high-risk conditions are ordered and discussed with the healthcare team.
* Collaborate with the healthcare team to provide clinical and technical support.
* Document all communications with healthcare professionals and patients in the medical record.
* Implement policies for the safe procurement, storage, distribution, and disposal of drugs and biologicals.
* Maintain all required documentation in compliance with federal and state regulations.
* Report medication-related errors to the Pharmacy Manager.
* Assist facility staff with safe and effective drug procurement and administration procedures.
* Perform other tasks as assigned.
Qualifications
* Bachelor’s Degree in Pharmacy; Pharm D preferred.
* One year of experience in long-term care.
* Licenses/Certifications: As required by government regulations and business needs.
* Detail-oriented, goal-driven, and a team player with strong oral communication skills.
