Overview
An opportunity has arisen for an additional full time Social Worker within the Preparing for Adulthood Team (PFA). We are looking for an enthusiastic, highly motivated, experienced Social Worker. You must have a Social Work Qualification and be registered by Social Work England.
Role Summary
This is an exciting opportunity to become part of a multi specialist team offering assessment and commissioning support for young people as they enter adulthood. You will work in a strength‑based way and aim to make a real difference to the lives of young people experiencing learning disabilities, physical disabilities, mental health and ASD.
Key Responsibilities
* Provide assessment and commissioning support for young people entering adulthood.
* Work in accordance with legislation such as The Care Act, Mental Capacity Act, and Mental Health Act.
* Communicate effectively, manage time, and use IT skills.
* Safeguard adults and collaborate in multi‑agency procedures.
* Support young people in achieving their aspirations and outcomes as they move into adulthood and adult services.
* Collaborate with Children’s Social Care, education, and health sectors.
Qualifications
* Social Work Qualification and registration with Social Work England.
* Experience in safeguarding adults and work within multi‑agency procedures.
* Knowledge of legislation: Care Act, Mental Capacity Act, Mental Health Act.
* Good communication, time‑management, and IT skills.
* A full driving licence and access to transport (disabled employees may use taxis).
