Social Worker – Housing Outreach Service (Bridgend)
37 hours per week. The post is grant funded until 2031. This role will be based on the Bridgend Substance Misuse Social Work team and is a CTM funded position.
Responsibilities
* Engage with homeless people and relevant service providers in Bridgend.
* Participate in effective partnership working and engagement with statutory and voluntary organisations to deliver services to children, young people and adults.
* Work in co‑production with children, young people, adults, their carers and significant others in undertaking assessments and planning.
* Work to appropriate and relevant legislation and guidance.
* Maintain the highest professional standards, uphold the Social Care Wales Code of Professional Practice and promote good practice within the area of responsibility.
Assessment and Care Planning
* Respond to referrals appropriately.
* Undertake assessments in line with the Social Services and Well‑being Act (Wales) 2014, identifying outcomes and where required developing care and support plans, which are monitored and reviewed.
* Follow safeguarding and child protection procedures appropriately.
* Work collaboratively with other agencies adopting a multi‑disciplinary approach where appropriate.
* Ensure electronically held records, including case recordings, assessments, care and support plans and reviews, are completed and/or updated in accordance with departmental policy and procedures.
Requirements
* Enhanced with Adults Barred list criminal records check (DBS) required.
* Valid driving licence.
