Band 6 Physiotherapist
Job Introduction
The Reablement service is a short‑term service supporting and working with people in their own home or in Residential or Nursing Homes. The aim is to maximise and maintain independence through rehabilitation and support. After an initial assessment and consultation with the service user, a care plan and rehabilitation programme is drawn up. Service users are encouraged to take an active part in developing their plans and are supported to make informed choices regarding their care.
This is a part‑time role working 22.5 hours per week at St Martins Hospital – Kempthorne House.

Key Responsibilities
* Provide an evidence‑based physiotherapy service of a high standard, undertaking comprehensive assessments, reassessments, care planning, rehabilitation and therapeutic care in a community setting.
* Use clinical reasoning and evidence‑based practice to inform appropriate intervention and develop comprehensive discharge plans.
* Act as a role model for more junior staff and students.
* Provide professional advice and guidance to the team, contributing to day‑to‑day management, data gathering, analysis, reporting and recommended action.
* Assist in integrating health and social care practices, helping to break down barriers and enable joint working and shared pathways with other practitioners and teams to provide holistic care to patients.
Essential Qualifications
* Diploma/degree in physiotherapy.
* Membership of Chartered Society of Physiotherapy or evidence of other professional liability insurance to practice.
* Registered with HCPC.
* Evidence of continuing professional development (CPD) recorded in a portfolio, as recommended by HCPC.
* Previous professional experience working in health and social care within the physiotherapy discipline.
* Depth of knowledge of principles and practices within physiotherapy.
* Good working knowledge of typical work‑related systems, procedures, equipment and technology.
* Knowledge and understanding of the legislation that applies to care and health work.
* Good levels of IT literacy, including knowledge of service‑related systems.
* Experience in a people‑management role, including conducting appraisals and performance management tasks.
* Experience of developing team members and seeking ways to improve services.
* Able to interpret and explain a variety of routine and complex technical information to a variety of audiences.
* Evidence of ability to resolve varied problems using sound logic, taking into account relevant implications and demonstrating good decision‑making.
* Full UK driving licence and access to a vehicle.
Desirable Qualifications
* Working towards or holding a postgraduate diploma/degree in professional practice and/or management practice.
* Previous experience of monitoring and managing a budget and understanding information governance requirements in a care/organisational setting.
