Main Duties and Responsibilities
* Keep patients using our services as safe as possible by recognising the limits of the HSW role and working closely with Registered Professionals.
* Contribute to best clinical outcomes by using up-to-date skills and adhering to evidence based policies and procedures when implementing care plans.
* Ensure people using our services have a good experience by respecting, empowering and working in partnership with them when implementing and evaluating care plans.
* Demonstrate skills and knowledge to contribute to the growth and maintenance of key functions of the clinical area by carrying out assigned tasks, involving direct and indirect care to patients, under minimal supervision and as directed by the nurse in charge.
* Work under the direction and supervision of Registered Professionals.
* Work to the standards set in the Code of Conduct for Healthcare Support Workers and Adult Social Care Workers in England (Skills for Health 2013).
* Act as Allocated Nurse, observe, interact with patients and document in nursing notes.
* Recognise and respect patients as individuals with the same rights, needs and emotions, promoting dignity and self-esteem.
* Safeguard people by recognising and responding when an adult or child might be at risk from abuse, recognising own limits and escalating concerns.
* Ensure patients make informed decisions whenever possible and respect their choices.
* Assist qualified staff in assessing patients’ needs and implementing individual care plans.
* Become proficient in venepuncture, smoking cessation, health promotion.
* Participate in facilitation of group activities and run clinics.
* Facilitate complex case management cases.
* Maintain accurate, legible, comprehensive records of observations and report significant findings to the nurse in charge.
* Escort patients as required with due regard to safety and security.
* Participate in observation of patients as per Trust Policy and after appropriate instruction.
* Participate in supervision and appraisal processes for reflection and personal development.
* Be open and transparent about own practice, identify areas for improvement in safety or quality, and work closely with others to improve services.
* Maintain compliance with mandatory training requirements.
Person Specification
Essential Criteria
* Willingness to undertake Care Certificate and further training as required.
* Ability to understand and implement the Equal Opportunities Policy at appropriate level, promote anti-discriminatory and anti-racist practices.
* Ability to work in a 7 day a week service model.
Skills and Knowledge
* Communicate with multi-disciplinary team and use handover effectively.
* Maintain accurate, timely, concise and legible clinical records.
* Maintain electronic patient records.
* Use Microsoft Word to draft letters and reports.
* Adhere to Code of Conduct for Health Care and Support Workers and Adult Social Care Workers in England.
* Contribute to implementation of care plans and assess patients.
* Work under own initiative but follow directions of care plans and Band 4 staff or registered nurses.
* Obtain consent and understand Mental Capacity Act and Deprivation of Liberty Safeguards.
* Work well with team, communicate effectively, be kind, considerate, reliable and enthusiastic.
* Help others achieve objectives and deliver safe, effective care.
Desirable Criteria
* Basic knowledge of Mental Health Act 1983.
* Experience in community or hospital settings, volunteer sector, caring role.
* Experience in offender or secure care setting, mental health setting, secondary care ward setting.
Previous Experience
* Previous experience of working with a multidisciplinary team.
* Experience of working with general public, communicating with varied people.
* Previous experience in fast-paced environment.
* Previous experience in customer focused environment.
Education and Qualifications
Essential criteria
* Numeracy and literacy skills to keep accurate records and administer medicines.
* Spoken and written English to appropriate standard.
Desirable Criteria
* Relevant Level 3 qualification – advanced apprenticeship, A-levels, T-levels, NVQ.
* Care Certificate – supported to complete within 3 months of starting if not achieved elsewhere.
* Venepuncture.
* Patient assessments and observations.
* Smoking cessation level 1 or 2.
