The Advanced Practitioner will provide highly specialised care within the hospital setting to patients with chronic and acute conditions, utilizing advanced clinical knowledge that is evidence-based. The role involves functioning independently or within a clinical team, demonstrating competence complementary to the doctor's role in healthcare delivery.
Responsibilities include:
1. Undertaking independent comprehensive assessments and management of patients, considering physiological, psychological, and pharmacological factors, and making complex clinical decisions about patient management and outcomes, including listing patients for elective procedures and discharging patients when treatment is complete or optimal outcomes are achieved.
2. Regularly re-evaluating and developing the service according to needs and resource availability.
3. Providing expert advice, leading, challenging, and changing practice within the clinical setting to meet patient needs.
4. Accepting new and follow-up referrals for patients with chest pain and syncope.
5. Performing procedures such as ILR implantations, tilt table testing, carotid sinus massage, and other procedures like ascites and chest drainage for ambulatory heart failure services.
6. Undertaking clinical audits, research, and service development.
7. Acting as an advocate for patients, carers, and the organization.
8. Providing counselling and advocacy for patients with acute and chronic conditions.
9. Leading change, resolving conflicts, and acting as a facilitator and resource for staff and other professionals.
10. Monitoring and evaluating the role of the Advanced Practitioner in collaboration with managers and medical staff, adapting to changing needs.
