We have a team of Care Coordinators already in post and are looking for another person to join the team.
Main duties of the job
This role involves:
Communicating withpatients to ensure they understand their condition, care pathway and treatmentand ensure that they are receiving the most appropriate support.
Support patientsand practices to achieve various health checks and chronic disease reviews donein a timely manner. You will be co-ordinating booking and following up DNAs,etc.
Working veryclosely with the practices, care homes, multidisciplinary teams within the PCN.The role is pivotal in ensuring our patients receive the best possible care andservice.

Job responsibilities
Key responsibilities:
1. Undertake work inline with PCN directed priorities.
2. Proactivelyidentify and work with a cohort of people to support their personalised carerequirements, using the available decision support aids
3. Ensure regular andconsistent communication with the referrer regarding patient progress and anycomplications or guidance
4. Raise awareness ofhealth promotion and NHS health checks in practices
5. Lead of the Saferecruitment and induction of all seconded staff into the PCN.
6. Evaluate treatmentprogrammes that promote health and well-being
7. Co-ordinateclinics, liaising with the schedulers and contacting the identified patientswith appointments.
8. Manage patientinitiated calls for help/signposting etc, booking into named GP urgent careslots/telephone slots if necessary.
9. Document andmonitor aspects of patient co-ordination and service delivery, supporting datacollection and audit using the patient administration system
10. Demonstrate theability to recognise and respond appropriately when faced with a suddendeterioration or emergency situation, alerting the team or enabling a rapidresponse.
11. Dependant on workplans, there may be a requirement to work across different member practices andteams.

Qualifications
Essential
1. NVQ Level 3 or equivalent level of knowledge in office procedures
2. Demonstrable commitment to professional and personal development
Experience
Desirable
3. Experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field (including unpaid work)
4. Experience of supporting people, their families and carers in a related role (including unpaid work)
5. Evidence of working with IT systems
6. Experience of data collection, analysis and providing monitoring information to assess the impact of services
7. Experience of partnership/collaborative working and of building relationships across a variety of organisations
8. Experience of working with or in general practice
9. Working in a multi-disciplinary setting where influence and negotiation is required
10. Knowledge/familiarity with medical terminology
11. Vulnerable adults awareness
12. Experience of care of the elderly
