About the Team:

This is an exciting opportunity to join our LDA and Sensory Adult Social Care team in Keynsham as a Social Worker. You'll play a vital role in supporting adults with a learning disability, autism and/or sensory impairment, helping them navigate complex needs and accessing the right support.

You'll be part of a warm, collaborative, and forward-thinking team, working alongside a wide network of professionals including Community Learning Disability Teams, AWP Mental Health Teams and more. It's a chance to make a real difference in the lives of young people and their families.

About the Role:

As a Social Worker, you'll manage a diverse caseload and work with a high degree of autonomy, supported by an experienced Team Manager. This is a role where your expertise and initiative will be valued - and where your impact will be visible.

Your responsibilities will include:

* Leading adult care assessments and reviews under the Care Act 2014
* Designing personalised care and support plans
* Conducting Mental Capacity Assessments and Best Interests Decisions
* Managing risk through thoughtful, person-centred strategies
* Completing Section 117 Aftercare reviews and Continuing Health Care Checklists
* Leading on Safeguarding enquiries

About You:

We're looking for someone who is passionate about empowering adults with diverse needs and committed to high-quality social work practice.

You'll need:

* A recognised Social Work qualification (Degree, Diploma, CQSW or equivalent)
* Registration with Social Work England
* Experience supporting individuals with autism, mental health conditions, learning disabilities, or sensory needs and physical disabilities
* A Full UK Driving Licence and access to a vehicle
