About The Role Support Worker – Hospital Discharge Service (Sandwell)
Location: Sandwell (covering West Bromwich, Oldbury, Smethwick, Wednesbury, Tipton, Rowley Regis and surrounding areas)
Hours: hours per week, rolling rota (Part of a Job Share)
Salary: £, per annum pro rata (£ per hour)
Contract: Fixed Term Contract Until st December
Additional Requirement: Full UK manual driving licence held for a minimum of years and a willingness to drive service vehicles
We are looking for a dedicated Support Worker to join our -day hospital discharge service in Sandwell. You’ll work closely with hospital teams and partner organisations to help people return home safely after a hospital stay and maintain their independence.
You’ll be part of a supportive team that includes two Support Workers, two Coordinators, and a Service Manager.
What You’ll Do
1. Respond to referrals and complete welfare calls and home visits.
2. Attend hospital wards and discharge units to support patients.
3. Provide practical and emotional support to aid recovery and independence.
4. Assess needs, identify safeguarding concerns, and accurately record case notes.
5. Signpost individuals to appropriate services and community resources.
This is a hands-on, community-based role where no two days are the same. You’ll need flexibility and confidence working in different locations across Sandwell.
Shift Pattern
6. -day working week on a rolling rota ( long days + short day).
7. Long days: :am – :pm or :pm – :pm
8. Short day: :am – :pm or :pm – :pm
9. Includes one weekend day every three weeks: :am – :pm ( long days + short day on this week)
10. Rotas provided months in advance.
