Responsibilities
* Discharge Planning & Logistics: coordinating transfers to skilled nursing facilities, rehab centres, or hospice.
* Setting up home health care, physical therapy, or medical equipment (like oxygen or hospital beds).
* Linking patients with community programs such as Meals on Wheels or transportation services.
* Clinical Assessment & Intervention: evaluating a patient's mental health, support system, and living situation.
* Providing immediate support for victims of domestic violence, sexual assault, or child/elder abuse (including mandatory reporting).
* Identifying addiction issues and facilitating referrals to detox or recovery programs.
* Helping patients understand Medicare/Medicaid or apply for charity care for unpaid bills.
* Assisting with Power of Attorney (POA) and Living Wills so the patient's wishes are respected.
* Acting as a bridge between the medical team and the family when there is disagreement over the care plan.
