Job Purpose
* Specialise in the development and reviews of person‑centred assessments and support plans.
* Support the drive and embed a culture of person‑centred practices that meet our mission, vision and values.
* Coach and mentor colleagues on person‑centred practices, identifying areas where existing plans can be strengthened.
* Support colleagues to think creatively about overcoming barriers to help people live amazing lives with increased independence.
* Embed an outcomes‑based culture to positively impact more lives.
* Deliver person‑centred practice workshops to colleagues across the charity.
* Identify areas of development and plan workshops or support sessions to drive quality improvements.
* Respond to challenges faced by people we support and help colleagues effectively respond by developing pathways, assessment tools and quality measures.
Key Accountabilities and Responsibilities
* Work closely with colleagues to complete person‑centred assessments. This includes working with internal and external professionals to ensure assessments lead to robust quality support plans that improve the quality of life of people we support.
* Upskill colleagues in conducting quality assessments and reviews, and in developing person‑centred support plans with meaningful outcomes.
* Work and communicate directly with people we support to ensure they are at the centre of the development of their assessments, support plans and outcomes.
* Engage with families, friends and professionals involved in the person supported’s life to gather a wide breadth of detail that informs the assessment and support plan.
* Remain involved and liaise with people supported and their important others during the first 12 weeks (family, advocate, social worker) to ensure that the support offered meets their needs in a person‑centred, outcome‑focused way.
* Lead a 12‑week review after admission to sign off or amend the package of care and support.
* Keep up to date with local and national policy guidelines in the sector.
* Develop and deliver with passion, sessions on person‑centred assessment and support planning principles.
* Review current support plans and identify areas for improvement, then share findings with team leaders and the support team to support colleagues in understanding why each area is important.
* Set realistic and achievable action plans to improve the quality of support plans.
* Drive cultural change to embed person‑centred practices that reflect our mission, vision and values.
* Write reports and presentations to reflect completed work, highlighting areas of focus and trends.
* Engage in continuous learning to enhance skills and competencies.
* Identify and develop pathways, assessment tools and quality measures for challenging areas (in partnership with quality partners).
* Support colleagues in the continuous development and improvement of operational practice.
Infection Control
All employees are required to be familiar with and comply with infection prevention and control policies relevant to their area of work.
Health & Safety
You have a legal responsibility not to endanger yourself, your fellow employees and others by your individual acts or omissions. The post holder is required to comply with the requirements of any policy or procedure issued in respect of minimizing the risk of injury or disease. All accidents must be reported to your manager in line with the general philosophy of the organisation.

Values and Attitudes
* Minimum of 2 years’ experience working in health and social care.
* Minimum of 2 years’ experience completing assessments and support plans.
* Experience of developing risk assessments and management plans.
Qualifications and Training
A Level 3 Qualification in Health & Social Care, or a Level 3 qualification in a related area.
Other
* Car Owner / Driver.
* Flexible with working hours and willing to travel, including overnight if required.
