Provide a proactive, trauma-informed, person-centred, and strength-based intervention service through individual safety planning and personal support.
Focus and prioritise service users who face additional barriers to accessing support and engaging in safety and support planning.
Deliver best practice housing and support services appropriate to clients' needs.
Attend regular one-to-one supervision meetings and team meetings.
Deliver clearly defined outcome targets for service users, ensuring quality assessments are maintained to the highest level.
Implement policies and procedures to ensure effective service delivery.
Promote the highest levels of service user consultation, communication, and partnership in all aspects of service delivery.
Ensure that development and access to education, training, and employment opportunities are central to the service.
Ensure key performance indicators (e.g., voids, arrears, and quality standards) are met within the sector's upper quartile.
Help maintain accurate and confidential case management records and databases, and contribute to monitoring information for the service.
Comply with data protection legislation, confidentiality, information sharing policies, and all relevant legislation.
Support colleagues and partner agencies through awareness raising and advocacy to provide the best service for users.
Respect and value community diversity, recognising the needs of a diverse range of service users to ensure accessibility for all.
Remain up-to-date and compliant with organisational procedures, policies, professional codes of conduct, and uphold standards of best practice.
Possess knowledge and understanding of providing support, advice, and advocacy to vulnerable clients, and effectively communicate this knowledge.
Have experience working with people with complex needs and delivering person-centred support within similar settings.
Maintain appropriate written and electronic case management records.
Adhere to all Local Solutions policies and carry out other duties as necessary or requested.
Have experience supporting service users who are homeless and accessing supported accommodation.
Hold qualifications in Youth and Community work, social work, HNC in Housing, Social Care NVQ Level 3, or equivalent.
