Job Summary
We are seeking a dedicated and compassionate nurse to join our Community Paediatrics Team, supporting the delivery of a high‑quality service for children, young people, and their families. This role focuses on providing health assessments, investigations, and treatment for children and young people with long‑term health conditions and neuro‑disabilities.
As part of this role, you will lead nurse‑led clinics for children with identified long‑term conditions, offer expert care, advice, and education, and support and mentor junior staff and learners to ensure safe, evidence‑based, person‑centred care.
Collaboration is at the heart of this position – you will work closely with multi‑disciplinary team members and partner agencies to ensure care is well‑planned, coordinated, and responsive to each child and family's needs.
Responsibilities
* Deliver evidence‑based nursing care and health assessments for children and young people with long‑term conditions and neuro‑disabilities.
* Manage an independent caseload, providing specialist advice, onward referrals, and clinical information to support Education, Health and Care Plans (EHCPs).
* Work collaboratively with multi‑disciplinary teams and external agencies to ensure effective care planning, safeguarding, and transition to adult services.
* Provide leadership, mentorship, and supervision to junior staff and learners, promoting professional development and maintaining clinical standards.
* Maintain accurate patient records and contribute to clinical governance, audit, and service improvement.
* Demonstrate flexibility and professionalism, promoting equality, diversity, and safety while adapting to changing service needs across multiple sites.
* Support and manage onward referrals to other professionals to ensure coordinated care.
* Conduct health assessments, screening, and redirection of referrals for children aged 5 years and over.
* Use evidence‑based practice to plan, implement, and evaluate care for children and young people with long‑term health and neuro‑disability conditions.
* Provide specialist clinical information and advice to support EHCP development.
* Work in accordance with the NMC Code of Professional Conduct.
* Maintain infection control, promote equality, diversity, and inclusion, and uphold exemplary standards of care.
* Provide guidance, supervision, and mentorship to junior staff and learners.
* Promote health and safety, identify, assess, and manage risks, and ensure compliance with Trust policies.
* Review and implement changes to patient pathways and care plans to improve outcomes.
* Maintain up‑to‑date mandatory training compliance and demonstrate flexibility during staff absence or changing priorities.
Systems and Equipment
* Maintain accurate, legible, and timely clinical records using the current electronic patient record system.
* Manage and utilise resources effectively, including staff, equipment, and supplies.
* Ensure appropriate data collection and reporting systems are in place and accurately maintained.
* Ensure safe use, cleaning, and maintenance of clinical and office equipment.
* Use visual display units (VDUs) and telephones as part of daily duties.

* Facilitate, develop, and deliver individual programmes and packages of care.
* Provide specialist nursing advice to the multi‑disciplinary team and contribute to clinical governance.
* Promote and implement evidence‑based practice to improve patient outcomes.
* Monitor and maintain standards of care through supervision, audit, and teaching.
* Reflect on own practice, identify development needs, and take action to meet them.
* Participate in annual appraisal, clinical supervision, and mandatory training programmes.

* Attend and contribute to core groups, case conferences, and multi‑disciplinary meetings, preparing statements and reports.

Physical Demands
* Travel between sites across Staffordshire as required.

Person Specification
Qualifications and Training
* Registered Nurse
* Evidence of continued learning within the speciality linked to clinical competence.
* Assessing and Mentoring qualification (ENB 998 or equivalent).
* Safeguarding Children and Young People Level 3.
* Desirable: Registered Nurse Learning Disability; Clinical Assessment qualification or equivalent; Non‑medical Prescribing.
Experience
* Minimum 3 years post‑registration experience in paediatrics.
* Experience working with children with a neuro‑disability.
* Proven experience of safeguarding children.
* Experience completing developmental monitoring of young people.
* Desirable: Experience working in community paediatrics, caring for children with complex medical needs, supervising junior staff, and working within the community with children.
Skills and Knowledge
* Paediatric physical health assessment skills.
* Knowledge of child development.
* Up‑to‑date knowledge of nursing issues, research, and evidence‑based practice.
* Understanding of government, local, and health issues related to specialist areas.
