General practitioner

Posted: 13 April
Offer description

The post holder will be required to support the Trust's strategy in developing and supporting the delivery of excellent emergency clinical care in West London. We are looking for someone who wants to work in a dynamic, innovative and evolving team. We are absolutely focused on ensuring that our hospitals provide high quality, safe and compassionate care, while driving forward the building of the new Hillingdon Hospital, which is due to be built by 2027. This is a full‑time post, but less than full time applicants willing to job share are welcomed. The Department focuses on evidence‑based emergency medicine, teaching, training and excellence, while maintaining operational standards.

A co‑located GP‑led Urgent Care Centre delivers care for both adults and children with minor illness and injury. There are stringent and well‑defined clinical criteria for streaming individual patients either to the Urgent Care Centre or Emergency Medicine. Patients may also be referred from the Urgent Care Centre to Emergency Medicine, where appropriate. The Emergency Department adheres to key performance indicators (KPIs) and the appointee will be expected to work toward these operational standards.

With this in mind, these posts will help achieve the operational standards while maintaining excellent care in the Emergency Department; furthermore, these posts comply with the new Specialty Doctor contract changes of 2021.

Education and training is at the forefront of the departmental philosophy. There is regular teaching and training opportunities, both informal (including bedside teaching from consultants, board rounds) and formal (including regular internal training days, journal clubs and secondments to other specialties) and these have contributed to the success of both trainees and Trust doctors in RCEM examinations.

Furthermore, the Emergency Department is committed to research involvement and quality improvement methodology in order to constantly improve the treatment delivered to our patient population.

Working for our organisation

The Hillingdon Hospitals NHS Foundation Trust has a thriving and expanding Emergency Department with a secure future which saw approximately 165,000 attendances in 2019‑19 (including attendances to our co‑located UCC). In addition approximately 100 patients a day are seen in the Minor Injuries Unit at our Mount Vernon Hospital site. The department has a history of innovation in patient care and has a continuing personal development programme to support on‑going service development.

Responsibilities

* Provide excellent direct clinical care for patients presenting to the ED, while working to maintain operational standards.
* Be responsible for the initial assessment, management and discharge or referral of patients to specialist teams.
* Provide timely and effective review of those patients initially assessed by the post‑holder (or handed over by junior or consultant colleagues), both in the main department and of those patients subsequently admitted to the Clinical Decision Unit.
* Provide clinical advice and support to FY1, Senior House Officers and nursing staff on the management of individual patients and ensure appropriate prioritisation of patient care.
* Assume the role of Trauma Team Leader when the Trauma team is activated.
* Assume the role of Team Leader for cardiac arrests occurring in the Emergency Department.
* Respond to a major incident call‑out and deliver clinical care in accordance with the Trust's Major Incident Plan.
* Be responsible, with the Nursing Shift Leader, for efficient floor management, including deployment of junior medical staff.
* Ensure that accurate, timely and sufficiently comprehensive information is communicated to patients, their carers and General Practitioners. This includes the clinical coding of every patient episode and the provision of discharge letters for all patients and for those sent to CDU.
* Liaise with colleagues within the Department, specialist teams providing inpatient care, the Clinical Site Practitioners, the Psychiatric Liaison Team, Rapid Response Service and the statutory, voluntary and other bodies involved in the provision of patient care within the community.
* Review pathology and imaging reports and initiate appropriate action as required.
* Participate in the Clinical Governance role of the Department, including quality improvement, risk management and the investigation of formal complaints and incidents.

Education and Training

* Clinically supervise and educate final year medical undergraduates attached to the Department of Emergency Medicine.
* Deliver regular teaching and induction programmes for Emergency Medicine SHOs.
* Provide educational activities for ED nursing staff, where appropriate.

Continuous Professional Development

* Continue own professional development in line with national guidance on revalidation as well as guidance from own professional body and the Royal College of Emergency Medicine.
* Comply with the Trust's mandatory training requirements.
* Maintain valid life‑support provider status for ALS, ATLS and APLS (or EPLS or equivalent).
* Actively participate in and contribute to the 'Senior EM Training Programme'.

Work Schedule and Contract

You will work a rolling rota with your Specialty Doctor and Specialist Registrar colleagues, delivering direct clinical care on a 24‑7 basis, with full internal cover for annual leave, study leave, and bank and public holidays. The rota will comprise 10 programmed activities per week: nine for direct clinical care and one for supporting professional activity.

Applications are welcome from all sections of the community to ensure a workforce that reflects our population.
